One of the common causes of pinched nerves in the low back and neck is disc degeneration. Disc degeneration is caused by wear and tear over the years with the disc getting thinner which narrows the vertebral spaces. This can cause the nerves to become pinched and irritated. This may then be followed with tingling and numbness in the shoulders and neck. Tension, stress, whiplash and other injuries can all contribute over time.

Sacroiliac joint pain may or may not actually be emanating from the joint itself. It could be simply situated in this region (buttock), ot it could be referred pain down the leg. It could be confused with myofascial type pain in the piriformis, gluteal muscles, or paraspinal muscles. Other patient disease processes that may mimic sacroiliac joint pain may include potentially painful or arthritic facet joints, tears in the outer part (annulus) of the intervertebral disk, all of which may radiate pain into the buttock area.

Options for spinal pain include traditional medications like over the counter analgesics or opioids, and also in between medications that are different from the traditional on-label medications. One of those medications includes gabapentin, which works well for neuropathic pain. Initially it was designed for epilepsy, but it is often used off-label for pain. Some antidepressants are often used for neuropathic pain issues.
